COVID-19 Screening

Before entering the Training Center, you must answer the brief quiz below.

Per our COVID-19 training plan, you must take this brief screening test within 24 hours before entering our building. Please note: this screening is NOT a COVID-19 test & DOES NOT determine whether or not you are infected with or have been exposed to COVID-19.

If you hesitate to answer any of these questions in a positive manner, please call us at (218) 733-9443 to reschedule your training. We will make every effort to work with you to keep all trainees safe and current on their training. Thank you.

If you pass this screening, you will see a green checkmark. Show us the green checkmark on your phone when entering the building to proceed to your temperature check. You will also receive an email with a copy of the green checkmark. You can also print or show us an electronic copy of that document to proceed.

If necessary, paper copies of this screening are available on request.

Health Quiz

1. In accordance with the Northern Mechanical & Iron Range Plumbers & Pipefitters Joint Apprenticeship & Training Committees COVID-19 Plan for the Training Center:
  • The number of visitors to the Training Center will be limited to only those necessary for the work.
  • All visitors will be screened in advance of arriving at the training center. If the visitor fails the health screening quiz or has a high temperature reading, he/she should not be permitted to access the training center.
2. Regardless of your vaccination status, have you experienced any of the symptoms in the list below in the past 48 hours?
IMPORTANT: ANSWER “YES” EVEN IF YOU BELIEVE THE SYMPTOM(S) IS BECAUSE OF SOME OTHER MEDICAL CONDITION (FOR EXAMPLE, ANSWER “YES” IF YOU HAVE A RUNNY NOSE BECAUSE OF ALLERGIES).
  • fever or chills
  • cough
  • shortness of breath or difficulty breathing
  • fatigue
  • muscle or body aches
  • headache
  • cough
  • new loss of taste or smell
  • sore throat
  • congestion or runny nose
  • nausea or vomiting
  • diarrhea
3. Are you isolating or quarantining because you tested positive for COVID-19 or are worried that you may be sick with COVID-19?

Have you tested positive for COVID-19 in the past 10 days?
4. Have you been in close physical contact in the last 14 days with:
  • Anyone who is known to have laboratory-confirmed COVID-19?
    -- OR --
  • Anyone who has any symptoms consistent with COVID-19?
Close physical contact is defined as being within 6 feet of an infected/symptomatic person for a cumulative total of 15 minutes or more over a 24-hour period starting from 48 hours before illness onset (or, for asymptomatic individuals, 48 hours prior to test specimen collection).
5. Have you traveled in the past 10 days? Travel is defined as any trip that is overnight AND on public transportation (plane, train, bus, Uber, Lyft, cab, etc.)
6. ALL VISITORS MUST REPORT TO THE APPRENTICESHIP OFFICE FOR THEIR COVID-19 & TEMPERATURE SCREENING. FURTHER ACCESS MAY BE DENIED PENDING THE RESULTS OF THE SCREENING.
7. WEARING OF FACE COVERING MAY BE REQUIRED.
8. YOU ARE NOT REQUIRED TO PROVIDE THIS DATA. HOWEVER, IF YOU REFUSE TO PROVIDE THE DATA, YOU WILL NOT BE ALLOWED ACCESS TO THE TRAINING CENTER.
9. I CERTIFY THAT MY RESPONSES ARE TRUE AND CORRECT.
If you are fully vaccinated AND answered NO to questions 2 and 3, and answered YES but fully vaccinated to Question 4, you are also acknowledging these important conditions for access to facilities:
  • Wear a mask in facilities for 14 days following exposure or until your test result is negative
  • Get tested 5-7 days after exposure
  • If you develop symptoms, immediately leave the facilities, isolate, and consult with your healthcare provider
